Before you can get your CDL, you need to pass a DOT physical exam. Here's exactly what to expect and how to prepare.
What It Covers
- Vision: 20/40 in each eye (corrective lenses okay)
- Hearing: Whispered voice from 5 feet
- Blood pressure: Should be below 140/90
- Urinalysis: Screens for diabetes/kidney issues (not a drug test)
- Physical exam: Heart, lungs, spine, neurological
- Medical history: Diabetes, heart conditions, sleep apnea, seizures
Common Disqualifiers
- Vision worse than 20/40 even with correction
- Insulin-dependent diabetes (waiver possible)
- Epilepsy with seizures in past 8 years
- Certain cardiovascular conditions
Important: Many conditions have waiver options. Don't assume — talk to a DOT examiner first.
How to Prepare
- Bring glasses/contacts if you wear them
- Bring a list of current medications
- Control blood pressure before the exam if it runs high
- Stay hydrated for the urinalysis
Cost & Validity
- Cost: $75–$150
- Valid for: 24 months standard · 12 months if blood pressure is elevated
- Where: Use the FMCSA National Registry to find certified examiners near you
Sleep Apnea Note
Sleep apnea is increasingly screened for. If diagnosed, you'll need to show CPAP compliance to maintain your medical certificate — but many drivers manage this without issue.
